Cherry Health Center respects your privacy and is committed to protecting your protected health information (PHI). This Notice describes how we may use and disclose your medical information and your rights under federal and Missouri law.

Our Responsibilities We are required by the Health Insurance Portability and Accountability Act (HIPAA) and Missouri law to:

  • Maintain the privacy of your PHI.

  • Provide you with this Notice of our legal duties and privacy practices.

  • Follow the terms of this Notice that are currently in effect.

  • Notify you if we cannot agree to a requested restriction on how we use or disclose your PHI.

How We May Use and Disclose Your Information We may use or disclose your PHI without your written authorization for:

  • Treatment — To provide you chiropractic, medical, physical therapy, BHRT, spinal decompression, or other care.

  • Payment — To bill your insurance, Medicare, Medicaid, or you directly.

  • Health Care Operations — For quality improvement, training, and business management.

  • Appointment Reminders & Health Information — To contact you about appointments or recommended services.

  • Required by Law — When mandated by court order, public health, law enforcement, or other legal requirements.

  • Business Associates — With vendors who help us operate (they are also bound by HIPAA).

We will obtain your written authorization for most other uses or disclosures, including marketing or sale of your information.

Your Rights You have the right to:

  • Request access to and copies of your PHI.

  • Request an amendment if you believe information is incorrect.

  • Request restrictions on certain uses or disclosures.

  • Request confidential communications (e.g., call you only at work).

  • Receive an accounting of disclosures.

  • File a complaint if you believe your rights have been violated.

Changes to This Notice We reserve the right to change our privacy practices and this Notice. Any revised Notice will be posted on our website and available upon request.

Complaints If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
